Our Program Mission: To improve the health and wellbeing of the White Earth Nation
As we grow and strive to bring programs to the people we serve, we provide flexible scheduling of home visits, educational offerings and events. To make a referral to any one of our programs, please call (218) 983 3286 between the hours of 8:00 am and 4:30 pm and ask for the intake nurse or if transportation is what you need, we do have dispatch available to assist with scheduling an available ride.

Ambulance
The mission of the White Earth Reservation Ambulance Service is to provide the highest level of pre-hospital care through rapid response, continuous training, compassion, and dedication to the people we serve.

Circleback Text-Center
The Circle Back Center intends to access the Consolidated Chemical Dependency Treatment Fund to cover the cost of care for eligible and authorized youth. Other funding sources may be available and are determined on an individual basis. There are no costs to referring agencies, individuals or families through the grant period ending on September, 2014.

The overall objective of the White Earth Home Health Agency is to provide client-oriented, efficient services, and education to individuals, families, and the communities of the Reservation. A variety of home and community based services are available through our agency.
The White Earth Home Health Agency accepts referrals from:
- Families
- Clinics
- Physicians
- Hospitals
- Friends
We are a state licensed, IV certified and Medicare certified agency. In fact, White Earth Home Health was the first Medicare certified tribal Home Health Agency in the nation!
To make a referral to any of our programs, please call us and ask for the Intake nurse. We would be happy to answer your questions.
Long Term Services And Supports
We provide a free visit within the home or nursing facility to help clients plan services and other needed supports that allow a client to remain in the home. Services, such as Personal Care Attendants to Life Line services are available to a client based on program eligibility.
Maternal Child Health
LIFE is a home visiting program designed to promote family self-sufficiency and to improve the health and wellbeing of children and families.
Harm Reduction Services
We assist clients with education on reducing harm related to preventable infectious diseases. By teaching healthier habits, we are able to reduce risk of community acquired infections. We also provide community and one on one Narcan trainings as a response to the nationwide opioid epidemic.
Community Health Representative (CHR)
Provides visits within the home to assist with personal cares, arranging medical appointments and assisting with medical forms. We provide health education on topics such as cancer and heart disease.
Transportation
We provide needed transportation to medical appointments, cancer treatments and dialysis. We have a centralized dispatch to assist with scheduling drivers based on location and client need.
Elder Wellness Program (EWP)
At its core we provide wellness by focusing on nutrition, self-awareness, physical activity and networking. EWP provides elders with information and resources on-site and throughout the communities to assist with goal setting, improving skill sets and addressing needs.
As a whole we provide the available programs: Anger Management, Public Health Emergency Preparedness, School Nursing Services, Child and Teen Check-ups, Immunization clinics, Foot care clinics, Podiatry clinics, wellness screenings and community education.
Substance Abuse
Our Mission: Combining the wisdom of yesterday with the knowledge of today we strive to provide each individual the tools to achieve manageability, to find happiness, fulfillment, and value in the gift of tomorrow.

Men’s Acute Care is a 9 bed, culture based facility located on White Earth Reservation. In 2019 we will be expanding and offering 20+ beds.
Men’s Acute Care serves men with substance use disorder. We are a medical assisted treatment program for those who are seeking MAT services.
